GENERAL DESCRIPTION

Since July 2014, CLSC Parc-Extension is a new family teaching unit able to accommodate full-time family medicine residents.Ìý Education is multidisciplinary and can be customized. The team is small, and the members include family physicians, nurse practitioners, clinical nurses, social workers, interpreters and administrative staff, all working in a collaborative and welcoming atmosphere. There are also several specialists (psychiatrist, pediatrician, pedopsychiatrist) involved in teaching and modeling shared care clinics.

The residents have their practice base at the CLSC, with academic core teaching at CLSC Côte-des-Neiges. Hospital rotations are at the À¦°óSMÉçÇø Health Center Hospitals, including the new Glen Campus Site and Jewish General Hospital.


POPULATION

CLSC Parc-Extension services the territory of the East End of Ville Mont-Royal, North End of Mile-End and the heart of Parc-Extension. The clientele varies widely socio-economical, and have heterogeneous background, especially around migratory experience. Walk-in services are open to all, every weekday. The Parc-Extension team has an expertise in pediatric care, mother-child health, palliative care, geriatrics and global health. The teaching office has a close collaboration with Maison Bleue Parc-Extension and residents are exposed and implicated in a multidisciplinary care model to target vulnerable patients.


OTHER INTERESTING INFORMATION

Residents will be primarily responsible for a patient load of about 200 patients which they will follow for the two years of their residency. As well, they will follow several pregnant women from their first prenatal visit until their postpartum visit, including their entire families.
CLSC Parc-Extension has a pediatric program, specialised in development and the residents are working with a pediatrician and a specialized multidisciplinary team.

As well, the residents will be exposed to travel and STI’s clinics which are run regularly at the CLSC and will receive specific teaching in these areas.

Education is multidisciplinary and can be customized. The patient population is a vulnerable, multicultural group whose initial contact with the health system is often at this CLSC. The team is small, and the members include family physicians with a wide variety of interest, nurse practitioners, clinical nurses, social workers, interpreters and administrative staff, all working in a collaborative and welcoming atmosphere. CLSC Parc-Extension has shared care and transcultural clinics in pediatrics, psychiatry, pedopsychiatry in which the resident takes part with the consultant and the specialized team.Ìý Also, the residents can be in contact with a travel and STI’s clinics and receive specific teaching in these areas.
